Palliative and Hospice care

Here at Meridian we offer both Palliative and Hospice care for your loved ones.


Using an interdisciplinary group approach of hospice staff, (physician, nurse, social worker, hospice aide, volunteer, spiritual counseling, bereavement, counseling and others), Hospice provides palliative care in the patient's home, short-term inpatient care, mobilization, coordination of ancillary services and bereavement support.


We have a small patient to staff ratio that gives us the opportunity to offer a more nurturing environment for your loved one. Please contact one of our team member for more questions about how we can care for your loved one.

What is Hospice?

Hospice care is centered on the patient, family and caregiver.


What is Palliative care?

Hospice provides care, treatment and services to those individuals who are in the final stages of an incurable disease, so that they may live out their remaining days as independent and as comfortable as possible.


Palliative care is specialized medical care for people with serious illness. It focuses on providing relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family.


We have an on-call Registered Nurse for any health questions you have 24 hours, 7 days of the week. The on-call nurse can help schedule a home visit within 48 hours of your call if it can't be handled over the phone. It's the same number you would call during office hours at 847-543-0045.
